Support Coordination
We will work with you and your supports - formal and informal - to explore and implement the supports identified in your NDIS plan.
We will meet with you to develop a list of your priorities and link in your preferred service.
We will work to implement and review your supports.
Behaviour Supports
We will conduct a thorough assessment and work with you and your supports.
We practice Positive Behaviour Support using the IABA Model.
Our process begins by defining behaviours and working to understand the purpose for the behaviours.
We will work to substitute the problematic behaviours with more socially appropriate ones.
Therapeutic Supports
We will complete a comprehensive assessment - looking at a range of life areas.
This assessment will inform us how best to work with you and look to improve your participation in appropriate social activities.
We will work with you to develop and implement your plan, reviewing progress toward your goals.
We will work with with services (e.g. Community Corrections) and assist you to meet your goals identified through the assessment process.
Support services are design to help people with disability live more independently and achieve their personal goals.
