Support Coordination Referral Form
Support Coordination Statement
Support Coordination assists people to understand their National Disability Insurance Scheme (NDIS) plan, assist to explore and implement their individual support needs and link people into services.
The Process
A support coordinator will work with the person and discuss how to use their funding from their NDIS plan to best achieve their goals by:
Assessment
• Assessing the person’s needs identified in their plan
• Assessing the person’s community options and providers to be linked into
Planning
• Choosing the best options for the person
• Arranging assessments that may be required at home or within services
• Looking at budgets needed for supports
Implementation
• Linking the person into services
• Working with chosen provider(s) and discussing needs, pricing and service
agreements
• Working with the person and their support networks to be able to self-manage
their plan and funding
• Working with NDIA planners around attaching the correct amount of funding
Evaluation
• Assisting the person for their plan review by assessing whether or not goals were achieved and what can be improved

Our Privacy Policy (available upon request) contains information on how you may request access to, and correction of, your personal information and how you may complain about a breach of your privacy and how we will deal with such a complaint.
INRS Support Services needs to collect information about you for the primary purpose of providing quality treatment. In order to fully assess, diagnose and treat you, we need to collect some personal information from you. This information will also be used for the administrative purposes of running the practice such as billing the NDIS.
INRS Support Services may disclose information regarding diagnosis or treatment to your doctor or specifically detailed services or other treatment providers only with your consent. INRS Support Services will not disclose your information to commercial companies, however, specific service or product information as deemed suitable for your management, may be forwarded to you by us, unless you instruct INRS Support Services not to forward this type of information. Your written consent will be obtained at the start of your treatment in order to carry out the above activities. We do not disclose your personal information to overseas recipients.
Information at INRS Support Services is stored securely and only INRS Support Services staff has access to it. INRS Support Services takes all reasonable steps to ensure that information collected about you is accurate, complete and up to date. You may have access to your information on request and if you believe that any of the information is inaccurate, we may be able to amend it accordingly. If you do not provide relevant personal or health information, in part or in full, to INRS Support Services it may result in incomplete assessment. This may impact on the diagnosis and the following therapy or services that is provided.
